Community Health Worker-In-Action
The Multicultural Health Foundation’s (MHF’s) mission is to bring health justice and wellness to the multicultural communities of San Diego county by focusing resources on the most vulnerable populations with community-based wellness strategies, social-clinical interventions, and research that will lead to the elimination of racial and ethnic health disparities. The MHF has a history of firsts in deploying a multi-lingual community health worker network providing chronic disease outreach services to the most vulnerable, African Americans/Blacks in Southeastern San Diego.
During the COVID-19 pandemic, Multicultural Health Foundation (MHF) was awarded the Community Health Worker Outreach and Engagement Contract from the County of San Diego.
Over the past year and a half, MHF implemented and managed a comprehensive and culturally sensitive community educational and outreach campaign in conjunction with trusted messengers and subcontracted partners. These messengers invested the time, resources, and good-will in understanding the unique strengths and challenges of the community
in order to improve the quality of life and health outcomes in the community of focus.
MHF promoted COVID-19 prevention activities to African American/Black populations at increased risk through virtual sessions, faith-based video and audio messages, professional and civic organization virtual speaker’s bureau, and neighborhood or affinity group virtual connects. As MHF leadership learned from CHW’s and the African American/Black community, additional educational outreach methods were employed to successfully reach the population.
