Celebrating 10 Years of service to the San Diego Community (2012-2022)

For the past decade, Multicultural Health Foundation’s (MHF’s) has been actively engaged in fulfilling its mission of bringing health justice and wellness to the multicultural communities of San Diego county by focusing resources on the most vulnerable populations with community based wellness strategies, social-clinical interventions, and research that will lead to the elimination of racial and ethnic health disparities.

The Business of Health Care

Healthcare is big business. That’s why in 2012, the Multicultural Medical Group (MMG) created the Multicultural Health Foundation (MHF) as its corporate social responsibility initiative to apply business of healthcare methods to design and finance prevention services.  MHF has raised over $2.5 million dollars to disrupt business as usual in the fight against chronic diseases by reinvesting cost savings, contracts with health providers, social health franchising and community-based entrepreneurial initiatives.

Although the MMG has retired its clinical services, its legacy continues in MHF’s trailblazing partnerships with the Alliance Healthcare Foundation, Center for Medicare and Medicaid, Be There San Diego, Scripps and Mercy Hospitals, Kaiser Permanente, American Heart Association and other insurance plans, clinics and community organizations to escalate high volume participation in prevention solutions.

We are forever grateful for Drs. Hood and Butcher and MMG’s conviction that every member of the community must reach their full health potential, no matter where they live, their skin color or income level.

 

Prevention and Wellness Solutions

San Diego County COVID-19 Equity Task Force

The San Diego County COVID-19 Equity Task Force is an innovative volunteer community-based coalition of stakeholders organized to discuss, advocate, and help implement policies and strategies to address the disproportionate negative impact the Coronavirus disease is inflicting on the diverse and vulnerable populations in the San Diego community.

Community Health Worker Community and Outreach Services

This is a comprehensive and culturally sensitive COVID-19 communication and outreach project using community health workers, radio, TV, billboards and social media to increase participation in testing, treatment, tracing and social distancing. Patient Health Improvement Initiative

The Patient Health Improvement Initiative works in a special way to help chronically ill patients in communities in San Diego County with the highest disease burdens and highest death rates. Once an individual meets the enrollment criteria, a team of medical and social specialists begin working closely with him or her.  

Prevention Services

The Prevention Alliance prevents the spread of chronic diseases like diabetes in multi-ethnic, immigrant and refugee communities.Free Prevention Workshops teach prediabetics how to use nutrition and weight loss to reduce the risk of becoming diabetic. The PA works with clinics, employers and insurance plans to offer Prevention Workshops to their members.  

Analysis and Action

The Health Equity Action Think Tank will use the data from the work with multicultural patients to understand their clinical and social challenges and share culturally appropriate interventions for their long-term wellness.

Healthy Reinvestment

One medical group saved more than $100,000 on participating patients and re-invested a portion back into the project so they could continue to receive clinical-social support and avoid regressing to medical crisis.
